Please provide your full name, the last 4 digits of your SSN, a phone number to reach you with any questions, and details of exactly what you want to change. If you want to contribute to the FSA or HSA, please email ftsi@myannualenrollment.com before 3:00 pm ET on Friday April 6th. Include your full name, last 4 digits of your SSN, a contact number for any questions, and specifics of what you want your 5/1/18 â 12/31/18 contribution to be. If you would like to enroll in any Unum Voluntary Benefits, please email ftsi@myannualenrollment.com before 4:00 pm ET on Friday April 6th. Include your full name, last 4 digits of your SSN, and a contact number for any questions. We will do our best to stay on schedule. Please help us do so by being available by your phone at the time of your enrollment appointment. In an effort to be courteous of everyoneâs time, the Benefit Counselor will have a âhard stopâ at the end of the scheduled appointment time frame in order to be available for their next scheduled appointment.
